The grainyness and the blur adds to the photo. The second head in the hair doesn't seem to do much for the photo, I would probably crop down to the very top of the forehead. The contrast is also nice and adds to the feel, but I think that the outer edge of the forehead may be a bit too bright, and takes away from the dark feeling. Great abstract work. :)
