There are 2 very dim softboxes... One to her right, one to her left, a coupld of feet behind the chair. And one 18-degree spot light on a boom at about 12 feet in the air pointing straight at hair...
Not an easy setup... Takes a lot of tweaking but the effect is exactly what I wanted.

I saw this one on your website last night and wondered why it hadn't been posted yet. It's another great shot. It has great color and comp. Oh and a wonderful model. On your lighting, I see one in front, high and to the right of the model and one that looks maybe behind her, straight over the chair but where is the other?
