It would be hard to judge what was used more in creating this image Your skills and the camera or Photoshop. It is a lovely illustration, don't get me wrong, but this is usefilm and not usephotoshop site. Therefore I have a feeling I should be conservative in writing about Your picture. In my opinion the position of the camera and the model are great, but as far as exposure and lighting this could be easily manipulated in post production.
